- x3 dropped by killing The One Reborn
- x3 in the chest behind the copper door filled with Labyrinth Watchers in Layer 3 Great Isz Chalice
- x2 in a chest before the boss room in a side area in Layer 2 Great Pthumeru Ihyll Chalice (there is also a crossbow-wielding labyrinth warrior dropping it randomly in the same layer in the main area with the fountain)
- x2 in a coffin in pre-area of Layer 3 Great Pthumeru Ihyll Chalice, after the Headless Bloodletting Beast fight.
- Can be bought from Insight Messengers in NG+ for 3 Insight
- Dropped by the Labyrinth Warriors sometimes encountered in pthumerian dungeons (a common drop by the two flaming sword Labyrinth Warriors in Layer 1 Pthumeru Ihyll Chalice (fetid/rotten/cursed - glyph: swjteujh) with eye rune(s) equipped.

AnonymousThere are also two in a coffin in Mergo's Loft in the Nightmare of Mensis. After defeating the Hunter on the Bridge (Choir Intelligencer Edgar) and entering the next room take an immediate left and open the coffin.

Hey there, I made a root chalice which contains a few chests with yellow backbones in them. The glyph is: u6xxzg9m. It is a Pthumeru Ihyll with no additional rites.

Great Isz Chalice.. Layer 3, after yellow lantern bonus room. 3x pcs of it in chest , it's a short journey compared to the pthemeru.. Hope it helps

There's two in a chest near the Mergo's Loft (base) lamp. Take the elecator up and go up the stairs to the left. The chest is in small side part of the room, where the columns are, to the right.
