Eye Pendant is a Key Item in Bloodborne. It was added with The Old Hunters DLC.
Eye Pendant Usage
- Used to activate the altar elevator to the Research Hall.
Location
- In The Hunter's Nightmare, inside the Nightmare Grand Cathedral, held by Laurence (Flaming Cleric Beast). He does not aggro if you pick up the item.

Interesting how the 3 main things on the Surgery Altar make a triangle: Left-Book, Right-Bell, Bottom-Beast and the very fact that this altar is in fact an elevator used for ascension to research hall.
